Behavioral Health Counselor: One Full Time position in the Waukon Medical Clinic. The Behavioral Health Family Counselor will work collaboratively as an allied team member of the clinical team to provide patient-centered care. The BH Family Counselor is an essential care team member and will be active in implementing best practices in the care of all patients This position will be responsible for maintaining accurate and concise records for client activity, conduct counseling services as directed by the patient’s primary care provider or the patient’s behavioral health specialist, will evaluate the need for and provide emotional and spiritual support to clients and families as well as conducts individual counseling sessions for patients as indicated by the treatment plan. Education: Licensed Mental Health Counselor or Marriage and Family Therapist (MFT), or Licensed Social Worker with a master’s degree from an accredited school of Social Work. Education: Minimum of an associate degree in nursing. Graduate from a certified RN program required. Two years of relevant experience or comparable combined experience required. Experience in a Rural Health Clinic preferred. Active RN license in the State of Iowa or ability to receive prior to starting date. Current CPR certification required. Current Iowa License or ability to obtain an Iowa license before the start of employment. Adult and Pediatric training are required and experience with all age groups preferred. Additional training is preferred in the following areas: Trauma-informed, Eye movement desensitization and reprocessing (EMDR) therapy, Therapy for personality disorders, Dialectical behavior therapy (DBT), Cognitive processing therapy (CPT).
Certified Medical Assistant ( Waukon Clinic):Â The Certified Medical Assistant (CMA) will work closely with Providers offering patient-centered care. They will assist with the planning, delivery, coordination, and tracking of patient care, prepare the patient for the exam, assist the Provider during the exam and treatment of the patient, and assist the patient after the exam to ensure understanding and compliance with Provider instructions. The Certified Medical Assistant will perform routine laboratory procedures including collection and preparation of specimens. This is a Part time position. Education: Completion of an accredited Medical Assistant program ,Certified Medical Assistant , Â A minimum of one year of health care experience in a primary care clinic preferred.
Dietary Cook: One PRN cook position available on an as-needed basis. Responsibilities include food/meal preparation, checking diet orders, patient tray set up and delivery and other duties as assigned. Previous food service experience and knowledge of sanitation requirements and modified diets is preferred.
Emergency Medical Technician (EMT) ER Tech: One PRN (as needed) position for the split role of EMT and ER Technician in the hospital’s Emergency Department. Job duties include a variety of EMS response types maximizing the use of EMS skills in the ambulance, in addition to direct patient care, registration and other required documentation pertinent to the Emergency Department. Current Iowa EMT license or NREMT certification and valid driver’s license with knowledge of basic practice relating to patient care required. Monthly formal EMS education credit provided with paid attendance of monthly meetings in addition to competitive wages, flexible hours, and complete hospital benefit package. For more information, contact Jacob Dougherty, Paramedic, EMS Manager or Callie Thorson, ER Manager at (563)568-3411.
ER Provider (Nurse Practitioner, ARNP or Physician Assistant, PA) – One full-time, 36 hour per week position available providing emergency care in the hospital’s ER. Must have at least two years’ experience in emergency department setting providing care to all patients who present in the ER. Qualifications include current Iowa ARNP or PA licensure with excellent clinical, organizational, communication, team and leadership skills.  Certification requirements include BLS, ACLS, PALS, NRP, and ATLS.
Home Care Aide: One PRN  position available. Responsibilities include providing all personal cares, homemaking services, running errands, and documentation of any changes in patient’s health condition or home environment among other duties. Must possess a valid Iowa driver’s license. Current certified nursing assistant license and previous nursing assistant, homemaker or home care experience preferred.
Hospital Provider: The ideal candidate for the Hospital Provider will be a graduate of an accredited Nurse Practitioner Program or Physician Assistant Program. The Nurse Practitioner or Physician Assistant Hospital Provider will provide general medical care to hospitalized patients. This position will coordinate care for inpatients by managing day-to-day care and providing prompt and complete attention to all patients care needs including diagnosis, treatment, and the performance of medical procedures that are within the Providers scope of practice. Facilitate collaboration, communication and coordination with the patient’s PCP and healthcare personnel included in the patient’s care. Additional duties and responsibilities include admitting and discharging patients from the hospital, reviewing Swing Bed requirements for admission, complete Swing Bed admissions and discharges, attend weekly staff meetings, enter orders into EMR system, completes medical reconciliation, performs chart reviews using Clinical Documentation Improvements (CDI) skills., Assist in ER & Urgent Care as needed as well as other duties and projects as assigned. Education and /or Experience: Current Iowa Registered Nurse Licensure, Current Iowa Advanced Practice Registered Nurse (APRN) Licensure, Completion of an accredited NP or Physician Assistant program, Current Nurse Practitioner certification in Primary Care, Emergency Medicine, or Critical Care, Active DEA License, current provider in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) ATLS – Preferred; Minimum of 1-2 years’ experience in Primary Care, Emergency Medicine, or Critical Care, two years’ experience with hospital based medical experience.
Physical Therapist – The Physical Therapist will be performing outpatient and home health physical therapy services. They will assess, treat, and monitor patients; perform all necessary paperwork and reports, plan and develop new programs; maintain communication with patient, patient’s family and physicians and contract site staff. This is a full-time position Education Requirements: Bachelors (B.A., B.S.), master’s degree (M.A., M.S.) or Doctorate (DPT) in Physical Therapy Experience. Additional Requirements:  Physical Therapy board certification and licensure according to state regulations. Supervisor’s license as required by the state Valid Driver’s license.
Radiologic Technologist: One Full-time position in our Radiology Department. Position responsibilities include but are not limited to, performing radiographs on patients according to Doctor’s/Radiologist’s orders, performs other procedures as needed including mammography, CT, MRI, bone densitometry, fluoroscopy, and other related services. Demonstrates good working knowledge of radiation protection, patient transportation, and sterile procedures. Qualifications: associate degree in Radiology or equivalent, registered/registry eligible by the American Registry of Radiologic Technologists (ASRT). Must possess and maintain a State of Iowa Permit to Practice. Clinical training as required by the State of Iowa to obtain work a permit. Previous hospital experience in general radiographic procedures is preferred.
Registered Health Information Technologist: The Ideal candidate for this position will work closely with the Health Information Services Coordinator, Medical staff, Financial Services, Nursing, other patient care departments and outside agencies to ensure complete and accurate patient records. Responsibilities of this position include but are not limited to, completion of coding using ICD 10 and CPT codes, abstracting information into the hospital-wide computer system, assigning codes for reimbursement and other duties as assigned. This position is full time Monday – Friday. Education: Must be a graduate of an approved school and hold a current Iowa certification as a Registered Health Information Technologist. Experience: Previous RHIT experience in a hospital setting is preferred.
Registered Nurse: One full-time RN position available working 36 hours per week. This position will require working rotating days and evenings and every third weekend. Candidates will also work two holidays per year. Current Iowa RN licensure required.
RN-Surgery: The Surgery RN position is 36 hours per week. Responsibilities of the surgery RN include providing coordinated nursing care to same day surgery patients; including admissions, surgery preparation, post operative care and discharge. The candidate will meet all licensure requirements necessary to maintain a current nursing status, will me Graduate of an accredited nursing program. Current RN licensure/eligibility in the State of Iowa required as well ACLS.
Registration Specialist – The Registration Specialist must possess excellent communication and customer service skills as well as the ability to show professionalism. The registration specialist is responsible for scheduling initial and follow up Provider appointments and any ancillary appointments, answering all incoming phone calls to the hospital and greeting patients and guests as they enter the hospital The registration specialist will ensure that all required forms are completed as part of the patient registration process. The registration specialist will also be required to take patient payments via cash/check/credit card. This is a part time position. Education Requirements: Minimum high school graduate or equivalent preferred. Experience: Previous receptionist or registration experience preferred.
Resource Nurse (Postville): One Full Time Registered Nurse in the Postville Medical Clinic. This candidate will work collaboratively as an active member of the clinical care team to provide patient-centered care. The RN will systematically and continuously collect and assess data related to the patient health status to develop, execute, and evaluate the plan of care for clinical patients. The RN will communicate effectively with patients to report any test results, schedule additional appointments, and be a liaison between the patient and the Providers. The RN will assist with direct patient care and be expected to be a leader in the growth of the organization.
Ultrasonographer: Â One Part time/PRN ultrasonographer to perform high quality diagnostic medical sonography exams and procedures to aide in medical diagnosis, treatment, and prevention of disease. Demonstrates accurate knowledge of established protocols and equipment operations when performing exams on patients. Performs diagnostic medical sonography procedures in the following areas: abdominal, OB/GYN, and vascular while providing the best images possible to radiologists and providers for interpretation. One year of ultrasound experience preferred and must be a Registered Diagnostic Medical Sonographer.Â
