Community and Home Care
VMH – Community & Home Care can provide medical treatment to help you recover and remain safe in the comfort of your own home. Our nursing, therapy and home care aid services can assist you with recovery from surgery, illness and/or injury, help you avoid unnecessary re-hospitalization and manage any chronic health conditions you may have.
Nursing Services
Our registered nurses will assess your condition and develop a plan of care tailored to meet your needs. This may include:
- medication set-up, administration and education
- dressing/wound cares
- injections and IV therapy
- catheter and ostomy management and education
- physical assessments
- patient and family care instruction
- communicating concerns to your physician
- providing recommendations to improve home safety
- assisting in coordinating medical appointments as needed
Home Care Aide Services
Certified Home Care Aides can help you remain safe in your own home by assisting you with:
- personal cares including bathing and dressing
- home exercise programs
- meal preparation and errands
- light housework
Therapy Services
Occupational therapists provide in-home evaluation and treatment to help patients develop and maintain the ability to perform essential activities of daily living such as bathing, dressing, preparing meals, etc.
Physical therapists evaluate and provide rehabilitation to home-bound patients following an illness, surgery, or injury. Physical therapy can include:
- exercise programs
- posture balancing training
- daily living assessments
- recommendations for home safety modifications
Speech therapists provide in-home evaluation and treatment for speech and swallowing problems including:
- assistance in producing speech sounds
- language comprehension and expression
- swallowing techniques
- diet recommendations
Insurance Coverage
VMH – Community & Home Care nurses will work directly with you to coordinate benefits and provide appropriate, cost-effective care to meet your needs. Listed below are examples of payer sources our agency utilizes.

Please feel free to call the VMH Community & Home Care Department at (563) 568.5660. Our office is located at 40 First Street SE, Waukon, Iowa 52172 on the upper level (3rd Floor) of the hospital. We are open from 7:30 am to 4:00 pm, Monday through Friday, and are on-call 24 hours a day, 7 days a week.
